LCMHCA Professional Disclosure Statement

I am grateful for the opportunity to work with you. This document provides information about my background
and experiences, and guidelines for our professional relationship. We will discuss any questions you have about
this document and our counseling relationship.
My Qualifications
I graduated from Wake Forest University with my Master of Arts in Clinical Mental Health Counseling in August
2020, after completing 60 credit hours. Wake Forest University is a Council for Accreditation of Counseling and
Related Education Programs (CACREP) certified program. I also hold a Master of Science degree in Forensic
Psychology from Arizona State University, completed in December 2020.
I passed the National Counselor's Exam and am a Nationally Certified Counselor. I am a member of the American
Counseling Association and the National Board of Forensic Evaluators.
I completed my Practicum and Internships at Anuvia Prevention and Recovery, giving me my counseling
experience. I have three years of previous experience in victim advocacy.
My North Carolina LCMHCA number is A16486. I am applying for my Licensed Clinical Addictions Specialist
Associate (LCASA) through the North Carolina Addictions Specialist Professional Practice Board. I will update
you when I receive approval and a license number.

Restricted Licensure
I am pursuing licensure as a Clinical Mental Health Counselor Associate in North Carolina. As I have a restricted
license, I am currently under the direct clinical supervision of Lori Harrow. Lori is a licensed counselor that
works with the Center for Emotional Health in Huntersville. Her office is at 16325 Northcross Dr., Suite F,

Counseling Background
My counseling is based on a Person-Centered approach. I want clients to be able to realize their potential in an
empathetic environment. I also like to utilize a Solution Focused approach for individual counseling. This
orientation allows the therapist and client to work together to accomplish any client's goals.
I have completed 13-hours of training from Strong Star Institute in Cognitive Processing Therapy (CPT). I have
completed 16-hrs of consultation and am working towards the goal of becoming a preferred provider in this
modality. Cognitive Processing Therapy is an effective therapy for PTSD.
I completed my 150-hour counseling practicum at Anuvia Prevention & Recovery as part of the Drug Court team.
I also completed a 600-hour internship with Anuvia as part of the drug court, the intensive outpatient programs,
and the SACOT program.

I am currently working as a contracted therapist with Center for Emotional Health in their Steele Creek office. I
am available for appointments Monday and Friday from 8am-5pm, and Tuesday and Thursday from 8am-2pm.
I am currently accepting and working with patients for individual outpatient therapy. I work with substance
recovery, trauma, chronic medical conditions, anxiety, depression, and other mental health disorders. I believe
that we can work together to create a plan to fit your needs best.
